Dear Prospective Volunteer:
Thank you for your interest in the Volunteer program at Northside Hospital. We now have three locations: Atlanta, Forsyth and Cherokee.
Please contact Northside Hospital-Forsyth or Northside Hospital-Cherokee if you are interested in volunteering at either of those locations. The following steps are required to become a Volunteer at the Northside Hospital –Atlanta location only:
· Complete the application form
· Sign the Agreement form and return this with the completed application
· Please note: Your application may not be processed immediately. You will receive a phone call to schedule an interview to determine your area of interest and the appropriate placement when openings are available.
· At this interview, be prepared to sign a Background Check consent form. You must have your own Social Security number and show your driver’s license when you come.
· As part of your health screening, two (2) TB skin tests are required and are given free of charge in the Employee Health office. The first one is given at the time of your interview and the second is given one week later.
All Northside Hospital Volunteers are required to be members of the Auxiliary, attend an initial orientation, purchase a volunteer uniform to wear while on duty, have an annual TB skin test, and pay annual dues. In addition, you are required to work your once a week four-hour commitment in your assigned service.
